In a herniation, the ring fails and core material migrates beyond the disc&#8217;s contour, where it can irritate or compress lumbar nerve roots \u2014 hence the leg pain (sciatica). The size of the herniation on MRI does not by itself decide treatment: the clinical picture does.<\/p>\n<h2>Common symptoms<\/h2>\n<p>Lower back pain radiating into the buttock and leg, tingling or numbness along the nerve, pain worse with sitting, bending, coughing or sneezing, and sometimes mild leg weakness.<\/p>\n<h2>Common causes<\/h2>\n<p>Progressive disc degeneration on a background of prolonged sitting, incorrect lifting, repetitive bend-and-twist movements, weak core support, excess weight and smoking.<\/p>\n<h2>Can it be treated without surgery?<\/h2>\n<p>In over 80% of cases, yes: active rehabilitation, stabilisation and centralisation exercises, postural hygiene and patience \u2014 radicular pain typically improves within 6\u201312 weeks.
