The causes differ greatly, and &#8220;generic&#8221; treatment disappoints \u2014 a precise functional diagnosis is the key.<\/p>\n<h2>What can shoulder pain mean?<\/h2>\n<p>Most commonly: subacromial syndrome (rotator cuff + bursa \u2014 pain raising the arm, &#8220;painful arc&#8221;), rotator cuff tendinopathy or tears (weakness, night pain), frozen shoulder (severe progressive stiffness), instability (after dislocations) and AC-joint or glenohumeral arthritis. Pain referred from the neck frequently imitates the shoulder.<\/p>\n<h2>Symptoms worth noting<\/h2>\n<p>Pain raising the arm between 60\u2013120\u00b0 (subacromial), night pain lying on the shoulder (cuff\/bursitis), stiffness limiting even passive movement (frozen shoulder), weakness on lifting (cuff tear) and neck pain with radiation (cervical).<\/p>\n<h2>Common causes<\/h2>\n<p>Repetitive overhead work, slumped posture, cuff\u2013deltoid imbalance, sudden overload, diabetes (a risk factor for frozen shoulder) and age.<\/p>\n<h2>Can it be treated without surgery?<\/h2>\n<p>The vast majority \u2014 yes: subacromial syndrome, tendinopathies, frozen shoulder and many degenerative cuff tears respond to structured exercise, with evidence comparable to surgery.
