An anterior cruciate ligament (ACL) injury is sport’s signature injury — and the road back, with or without surgery, runs through correctly conducted rehabilitation.

What is an ACL injury?

The ACL stabilises the knee against excessive forward translation and rotation of the tibia. It typically tears WITHOUT contact: a landing, pivot or sudden deceleration. Meniscus or collateral injuries often come with it. The surgery-versus-conservative decision depends on age, sport, instability and goals — made with the orthopaedist, on functional grounds.

Common symptoms

A felt or heard “pop” at injury, massive swelling within hours, instability (“the knee gives way”) with pivoting, and lack of confidence going downhill or on uneven ground.

Surgery or not?

Not automatically: patients with high rotational demands (football, skiing, basketball) tend towards reconstruction; patients with linear activities can do very well conservatively with a trained knee (“copers”). In BOTH scenarios, rehab quality decides the outcome — and pre-operative rehab (“prehab”) measurably improves post-operative results.

How does rehab proceed?

In phases with objective criteria: calming the knee and regaining full extension, quadriceps reactivation, progressive strength, running retraining, then pivoting and plyometrics, and finally return-to-sport testing (strength and hop symmetry) — at 9–12 months post-reconstruction, not “6 months by calendar”, because haste is the main cause of re-rupture.

How we work at Medicine Park

Coordinated with your orthopaedist: prehab, post-operative rehab or a full conservative programme, with objective testing at each stage, landing-mechanics work and re-injury prevention (the other knee’s risk is real). All in English.

How long does recovery take?

Post-reconstruction: 9–12 months to pivoting sport. Conservative: 3–6 months to safe linear activity. Stages are measured, not guessed.

When to seek help

Right after injury (prehab counts) or whenever a knee remains unstable after an old tear.
