About your condition

A joint that flares up suddenly in the night — red, hot and so painful that even the weight of the bedsheet is unbearable. That is how gout usually announces itself, most often at the base of the big toe. It is a form of arthritis that responds very well to treatment: the attack settles, and the mobility lost after repeated attacks can be rebuilt.

What is gout?

Gout occurs when uric acid builds up in the blood (hyperuricaemia) and forms crystals inside a joint. The crystals trigger intense inflammation that develops within hours. The joint at the base of the big toe is the classic site, but gout can also affect the ankle, knee, elbow or fingers.

Common symptoms

Severe pain coming on suddenly, often at night, usually in a single joint; a swollen, red, hot joint that is extremely tender to touch; shiny, stretched skin over it, sometimes peeling once the attack subsides. An untreated attack lasts from a few days to two weeks, and there may be months without any symptoms in between. After years of repeated attacks, tophi — small nodules under the skin — and lasting joint stiffness can develop.

Common causes and risk factors

The direct cause is raised uric acid. It is encouraged by purine-rich foods (red meat, offal, seafood), alcohol — beer in particular, fructose-sweetened drinks, dehydration, some medicines such as diuretics, excess weight, kidney disease and a family history. Men are affected more often; in women the risk rises after the menopause.

When to seek urgent medical care

A fever alongside a red, hot joint may mean joint infection rather than gout — that is a medical emergency. A first attack should also be confirmed by a doctor: a uric acid test and, where needed, examination of the joint fluid distinguish gout from conditions that mimic it, such as pseudogout, infection or bursitis.

Can it be treated without surgery?

Yes — gout is not a surgical condition. Treatment runs on two tracks, both set by your doctor: settling the attack with anti-inflammatories, colchicine, joint rest and ice; and lowering uric acid over the long term, alongside good hydration, limiting purines and alcohol, and managing weight. With the right treatment, attacks can become rare or stop altogether.

How we treat the after-effects of gout at Medicine Park

Gout is treated with medication — but what it leaves behind is rebuilt through movement, and that is our part. Once the acute episode has settled we work on joint stiffness and mobility with a progressive physiotherapy programme that does not overload the affected joint, reduce residual swelling with physical therapy and manual lymphatic drainage, retrain gait and loading when gout has affected the foot or knee, and build an activity programme you can sustain — regular movement and controlled weight both reduce how often attacks return. During an acute attack the joint is protected, not exercised.

Baths, bicarbonate of soda and home remedies — what actually helps

Warm baths may ease discomfort for a while and ice reduces inflammation during an attack. But no home remedy — bicarbonate of soda, vinegar, compresses — lowers uric acid or replaces medical treatment. Use them for comfort at most, never as a treatment.

What to eat and what to avoid

Limit: offal, red meat, seafood, beer and spirits, sweetened drinks. Favour: plenty of water, low-fat dairy, vegetables, wholegrains, cherries. Purine-rich vegetables such as spinach, mushrooms and tomatoes have not been shown to increase attack risk, so overly strict diets do not help. A personalised eating plan remains a matter for your doctor or dietitian.

How long until results?

Rebuilding mobility starts once the attack has settled and is usually noticeable within 3–6 weeks of consistent work. In joints affected by repeated attacks, the gains consolidate over 2–3 months and are maintained through regular movement.

When to seek help
