About your condition

If every step feels like you are walking on a pebble, or the ball of your foot burns after standing, you are likely dealing with metatarsalgia — overload of the small joints and soft tissues under the heads of the metatarsal bones.

What is metatarsalgia?

Metatarsalgia is not a single diagnosis but a pain pattern: the forefoot takes more load than it is built for, and the tissue under the metatarsal heads becomes inflamed and painful. It is common in runners, in people who spend long days on their feet, and in women who wear narrow or high-heeled shoes.

Typical symptoms

Sharp, aching or burning pain under the ball of the foot; pain that worsens when you walk barefoot on hard floors; a feeling of a fold or pebble in the sock; occasional tingling in the toes.

Why it happens

The real cause usually sits higher up the chain: a stiff big-toe joint, a tight calf that shifts load forward, weak intrinsic foot muscles, altered gait after another injury, or foot shape (high arch, long second metatarsal). Insoles alone often fail because they cushion the symptom without correcting the mechanics.

Treatment combines manual therapy for the stiff segments, progressive strengthening of the foot’s own muscles, calf and ankle work to redistribute load, and precise guidance on footwear and training volume. How long does recovery take?

Most mechanical forefoot pain improves meaningfully within 4–8 weeks of structured rehabilitation. The earlier we correct the loading pattern, the faster it settles — and the less likely it returns.
