About your condition

Diffuse pain around or behind the kneecap that flares on stairs, squatting and after long sitting with bent knees — at the cinema, the desk, in the car? That is the picture of patellofemoral pain syndrome, the most common cause of knee pain in young and active people.

What is anterior knee pain?

Patellofemoral pain is the suffering of the joint between kneecap and thigh bone: the kneecap does not glide optimally in its groove, so pressure on the cartilage rises and distributes incorrectly. It is not arthritis, and imaging can be completely normal — the problem is functional, which is exactly why it responds so well to functional treatment.

Common symptoms

Diffuse pain “around” the kneecap (patients show it with the palm, not a finger), worse on stairs (especially down), squatting, running and prolonged sitting with bent knees (the “cinema sign”), sometimes creaking and a feeling of insecurity.

Common causes

Weak quadriceps and especially glutes (they control knee alignment), sudden activity increases, inefficient running or landing mechanics, a stiff lateral chain and unsuitable footwear.

Can it be treated without surgery?

Yes — almost exclusively: the evidence puts targeted exercise first (quadriceps + glutes), load education and mechanics correction. Surgery has no place in treating the common form.

How we treat patellofemoral pain at Medicine Park

We assess the knee, hip, ankle and movement patterns (including landings and running where relevant), then build the programme: progressive quadriceps and glute strengthening, movement-pattern correction, manual therapy for stiff structures and intelligent activity dosing — no total rest. All in English.

How long does recovery take?

Typical improvement: 6–8 weeks of consistent work; consolidation: 3 months. Relapses come from abandoning the exercises — we build you a minimal maintenance routine.
