Hip or groin pain when you walk, climb stairs or get up from a chair? “You have hip osteoarthritis” — and your mind jumps straight to a hip replacement. Hip osteoarthritis (coxarthrosis) is wear of the hip cartilage. Here is the evidence-based reality: coxarthrosis progresses slowly, and in early and moderate stages a structured rehabilitation program can substantially reduce pain, protect mobility and postpone — sometimes avoid — surgery.

What is hip osteoarthritis?

Hip osteoarthritis (coxarthrosis) is the gradual wearing of the cartilage lining the hip joint. As cartilage thins, the joint becomes inflamed and stiff, and the surrounding muscles weaken — a vicious cycle that accelerates decline unless it is interrupted with the right loading.

Typical symptoms

  • groin or outer-hip pain, sometimes radiating to the knee;
  • morning stiffness, or stiffness after sitting;
  • difficulty with socks, shoes, getting out of a car;
  • a limp after longer walks; in bilateral cases, both hips are affected.

Do you need a hip replacement?

Not necessarily — and usually not yet. International guidelines put exercise therapy, education and weight management first. Lost cartilage does not regrow, but pain and function can improve substantially with the right program. A hip replacement is the solution for advanced stages that no longer respond — a decision made with your orthopaedic surgeon based on how you actually function, not on the X-ray alone.
