Groin pain is one of the most deceptive complaints in sports medicine: the pain sits in one place, but the source can be the adductor muscles, the hip joint, the pubic symphysis or even the lumbar spine. Identify the real driver, and treatment usually succeeds — without surgery.

Common causes

  • Adductor strains — the classic cause in football, running and change-of-direction sports;
  • Athletic pubalgia / osteitis pubis — overload of the pubic symphysis;
  • Hip joint problems — osteoarthritis and femoroacetabular impingement typically project pain into the groin;
  • Referred pain — from the lumbar spine or sacroiliac joint;
  • Non-musculoskeletal causes (inguinal hernia, urological or gynaecological conditions) — which is why a careful first assessment matters.

Typical symptoms

  • pain at the top of the inner thigh, worse with running or kicking;
  • pain when squeezing the legs together against resistance;
  • deep hip ache felt “in the crease”, sometimes with limited rotation;
  • pain that builds with activity and partially settles with rest.

Does groin pain need surgery?

Usually not. Adductor injuries, pubalgia and early joint-related pain respond well to structured rehabilitation. Surgery is reserved for specific confirmed diagnoses — and even then, active rehab remains part of the plan.
