About your condition

Deep groin pain that appears when squatting, getting into the car, during sport or after long sitting — in a young, active adult? Femoroacetabular impingement (FAI) is one of the “fashionable” modern causes of hip pain — and one where conservative treatment almost always deserves the first chance.

What is hip impingement?

A mechanical conflict between the femoral head/neck and the rim of the hip socket, due to shape variations (a “cam” on the femur, a “pincer” on the socket, or mixed). In flexion and internal rotation the structures “pinch” — irritating the labrum and cartilage. An important nuance: these bone shapes also exist in many people WITHOUT pain — shape alone is not disease.

Common symptoms

Deep groin pain with flexion and internal rotation (squatting, getting into a car, low seats), pain after prolonged sitting, limited internal rotation, sometimes clicking or sharp catches with certain movements, and discomfort in pivoting sports.

Causes and factors

Bone shape (partly genetic, partly developed through intense adolescent sport), overload in deep flexion (hockey, football, dance, loaded deep squats) and deficient motor control of the hip and pelvis.

Can it be treated without surgery?

Often, yes: studies show structured rehab (motor control, gluteal and trunk strengthening, modifying provocative ranges) significantly improves pain and function for many patients. Hip arthroscopy remains an option for non-responders with significant labral lesions — a decision made with the orthopaedist after a properly tried conservative programme.

How we treat FAI at Medicine Park

We assess the hip, pelvis, mobility and movement patterns, then build the programme: gluteal and deep-stabiliser strengthening, motor control of flexion, sport-technique adjustments (squat depth, positions), manual therapy for tight structures and measured progression. All in English.

How long does recovery take?

Typical improvement: 6–12 weeks of consistent work; consolidation: 3 months. We reassess honestly if progress does not appear.
