About your condition

Osteitis pubis is one of the most frustrating injuries in sport: a deep ache at the front of the pelvis that eases with rest, then returns the moment you push training again. Football players, runners and athletes who change direction a lot know it well.

What is osteitis pubis?

It is an overload reaction of the pubic symphysis — the joint at the front of your pelvis — and the tissues that anchor into it. Repetitive kicking, sprinting and cutting create shearing forces across this joint; when load exceeds what the tissue can tolerate, it becomes inflamed and painful.

Symptoms

Pain directly on the pubic bone or spreading into the groin and lower abdomen; pain with sprinting, kicking, twisting or even coughing; stiffness the morning after training; a gradual onset that athletes often ignore for months.

Why rest alone doesn’t fix it

Rest calms the symptoms but does not change the reason the joint got overloaded — usually a combination of hip stiffness, adductor and abdominal strength deficits, and training spikes. That is why the pain returns when you do. The evidence-based path back is a progressive loading programme, not prolonged rest.

How we treat it at Medicine Park

After confirming the diagnosis pattern and screening the hip and lumbar spine, we build a staged plan: first settle irritability, then progressively strengthen the adductors, abdominals and hip rotators, then rebuild running, change of direction and kicking tolerance step by step. How long does it take?

Recovery is typically measured in weeks to a few months depending on how long symptoms have been present. Athletes who start structured rehab early return significantly faster than those who cycle through rest and flare-ups.
